LIFE SKILLS COACH 1 - JOB DESCRIPTION

GENERAL RESPONSIBILITIES: A Life Skills Coach will support the overall development, implementation and supervision of each client’s Individual Service Plan (ISP), which will outline the clients’ trainings and target goals. These responsibilities include but are not limited to the following:

  • Using the concepts of active treatment, participate in the development, implementation and monitoring of each client’s ISP.

  • Be knowledgeable and supportive of the rights of the Developmentally Disabled.

  • Encourage each client/consumer to advocate for themselves and when necessary, serve as an advocate for the client.

  • Provide an environment that supports change and encourages informed choice for the client.

  • Attend required company meetings and training sessions.

  • Familiarize and adhere to the Policy and Procedures of Cypress Independent Living Services, Inc. and be familiar with Employee Personnel Handbook.

  • Monitor ongoing progress of client’s goals and be a direct contact with Regional Center Personnel and other members of our Client’s interdisciplinary team.

  • Attend scheduled meetings to review client progress as scheduled semi –annually.

  • Monitor client billable hours to ensure that all assigned hours are completed

  • Complete semi-annual reports as assigned that are reviewed during meetings

  • Maintain appropriate paperwork associated with individual clients as assigned


CORE RESPONSIBILITIES OF A LIFE SKILLS COACH:

  1. Ensure 100% of Assigned ILS Hours are Delivered to your caseload of Disabled Adults each week

  2. Document and plan for any Undelivered ILS Hours to be delivered by the end of the month.

  3. Turn in All Session Case Notes each week with your signature by the designated time

  4. Turn in All Time Records each week with your signature by the designated time

  5. Turn in your updated Monthly Schedule by the first of the new month – if any changes have occurred in your schedule, they must be submitted to your supervisor immediately.

  6. Communicate swiftly and thoroughly with your co-workers and outside agencies who rely on your information for the planning of their care for our joint clients. The Cypress ILS Quality Standard is within 24 hours.

MINIMUM JOB REQUIREMENTS:


  • Must be a minimum of 21 years of age

  • Must have a reliable vehicle which passes a vehicle inspection

  • Must have proof of full coverage insurance on your vehicle.

  • Vehicle must seat at least 4 adults with seatbelts

  • Must complete items outlined on the employee checklist (i.e. 1-9, W-4)

  • Prior to being hired, employee must provide proof of acceptable driving record (DUI on record will not be accepted). If an employee receives a DUI at any time during employment, they will be subject to immediate termination. Employees must also continue to maintain an acceptable driving record during their employment.

  • Employee must have CPR and First Aid. If they are not certified, Cypress Independent Living Services. Inc, will assist them with registering for the appropriate course

  • Must be able to communicate well verbally and in writing

  • Good organizational skills are a must. Candidates for this position must also be able to motivate others and function with minimal supervision

  • Must work well with clients, families, other professionals and the community at large

  • Employees will be responsible for establishing structured work schedules with Supervisor final approval required; employees may elect to distribute client monthly hours throughout the month. However, hours should be regularly scheduled at the same time each week.

  • Employees are responsible for ensuring they schedule a paid 15 minute break for every 4 hours of work scheduled, and an unpaid 30 minute lunch break for every 6 hours of work scheduled. A second paid 15 minute break is also to be scheduled if an employee works 8 hours in one day.

  • Employees are limited to a workday of 8 hours, and a maximum of 6 days in a row. No overtime will be authorized unless prior approval from management is obtained. Any unauthorized overtime will result in immediate disciplinary action up to and including immediate termination.

  • Employees are responsible for completing all assigned client monthly hours. If for any reason client hours will not be completed, employees must notify their supervisor immediately.

  • Employees understand that they must be able to pass a background check. Employment is contingent upon this, and an employee may be immediately terminated if they fail to pass the background check.
